SUBJECT: Approval of Contract Amendment Agreement #26-239-5 with
Carmen Valdivia
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #26-239-5 with
Carmen Valdivia, effective September 30, 1993, to amend Short Form
Service Contract #26-239-4 (effective July 1, 1993 through September
30, 1993) for Spanish language interpreter services at Richmond,
Pittsburg and Martinez Health Centers. This amendment increases the
Contract Payment Limit by $15, 000, from $7, 500 to a new total Payment
Limit of $22 , 500, and extends the term of the agreement through
December 31, 1993 .
II. FINANCIAL IMPACT:
This Contract is included in the Health Services Department's
Enterprise I budget.
Approval of this Contract Amendment Agreement will result in a
cumulative twelve-month total in excess of $25, 000, and therefore,
Board of Supervisors approval is required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In July, 1993 , the County Administrator's Office approved and the
Purchasing Services Manager executed Short Form Service Contract #26-
239-4 with Carmen Valdivia, to provide Spanish language interpreter
services to patients at Richmond, Pittsburg and Martinez Health
Centers.
Approval of Contract Amendment Agreement #26-239-5 will allow the
Contractor to continue to provide these essential translation services
through December 31, 1993 .

• AMENDMENT SPECIFICATIONS

In consideration of Contractor's agreement to provide additional
services under the Contract identified herein, County and Contractor
agree to amend said Contract as specified below, while all other parts
of the Contract remain unchanged and in full force and effect:
1. Extension of Term. The Term of the contract set forth in
Paragraph 3 . (Term) is hereby extended from September 30 , 1993 through
December 31, 1993 .
2 . Payment Limit Increase. The Contract Payment Limit amount set
forth in Short Form Service Contract Paragraph 5. (Payment Limit) is
hereby increased by $15 , 000, from $7 , 500 to a new total Payment Limit
of $22,500.
3. Increase in Number of Service Units. The number of service
units set forth in Paragraph 6. (County's Obligations) , subparagraph a.
is hereby increased from 559. 5 service units to a new total of 1, 679
service units.
