Spooner, Contracts Administrator Costa DATE: September 30, 1993 09 County SUBJECT: Approval of Contract Amendment Agreement #26-239-5 with Carmen Valdivia SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute on behalf of the County, Contract Amendment Agreement #26-239-5 with Carmen Valdivia, effective September 30, 1993, to amend Short Form Service Contract #26-239-4 (effective July 1, 1993 through September 30, 1993) for Spanish language interpreter services at Richmond, Pittsburg and Martinez Health Centers. This amendment increases the Contract Payment Limit by $15, 000, from $7, 500 to a new total Payment Limit of $22 , 500, and extends the term of the agreement through December 31, 1993 . II. FINANCIAL IMPACT: This Contract is included in the Health Services Department's Enterprise I budget. Approval of this Contract Amendment Agreement will result in a cumulative twelve-month total in excess of $25, 000, and therefore, Board of Supervisors approval is required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: In July, 1993 , the County Administrator's Office approved and the Purchasing Services Manager executed Short Form Service Contract #26- 239-4 with Carmen Valdivia, to provide Spanish language interpreter services to patients at Richmond, Pittsburg and Martinez Health Centers. Approval of Contract Amendment Agreement #26-239-5 will allow the Contractor to continue to provide these essential translation services through December 31, 1993 .
