Spooner, Contracts Administrator Costa DATE: September 23, 1993 County SUBJECT: Approval. of Contract Amendment Agreement #22-435-2 with Catholic Charities (Diocese of Oakland) SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute on behalf of the County, Contract Amendment Agreement #22-435-2 with Catholic Charities (Diocese of Oakland) , effective August 1, 1993, to amend Standard Contract #22-435 (as amended by Contract Amendment Agreement #22-435-1) , to increase the payment limit by $36,442, from $45,500 to a new two-year Contract payment limit of $81,942, and to extend the term of the Contract through June 30, 1994. II. FINANCIAL IMPACT: This Contract is funded 100% by Federal funds through the CARE Act, Title 1, and HUD "Housing Opportunities for People with AIDS" monies. No County funds are required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: In October, 1992, the County Administrator approved, and the Purchasing Agent executed, Standard Contract #22-435 with Catholic Charities (Diocese of Oakland) , to provide housing advocacy services for persons in Contra Costa County with HIV Disease and their families, for the period from August 1, 1992 through June 30, 1993 . Contract Amendment Agreement #22-435-1, approved by. the Board of Supervisors on June 22, 1993 , extended the term of the agreement through November 30, 1993 . Approval of Contract Amendment Agreement #22-435-2 will allow the Contractor to provide additional case management services, including emergency housing, food and transportation assistance, through June 30, 1994. (Progress Reports) of the Service Plan." 4. Modification of Service Activities. Paragraph 4. (Modification of Service Activities) of Contract Amendment Agreement #22-435-1 is hereby modified to read as follows: "2. Service Activities. Contractor shall provide HIV/AIDS Case Management services, including, but not limited to, emergency housing, food and transportation assistance, for Contra Costa residents diagnosed with HIV Disease, in accordance with the Work Plan which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite 200, Martinez, and which.is incorporated herein by reference." (Submission of Progress Reports) of Contract Amendment Agreement #22-435-1 is hereby modified to read as follows: "3. Reports. Contractor shall submit to County's Health Services Director, or his designee, quarterly written progress reports and monthly data reports, in the form and manner required by County. Said progress reports shall include, but not be limited to, the following: a. A description of Contractor's progress in conducting said HIV/AIDS Case Management services; b. A cumulative description of the work and services provided; C. Any difficulties or special problems encountered; d. Any pertinent facts or interim findings; and e. A statement that the Contractor is or is not progressing satisfactorily in conducting and completing the services. Contractor shall submit on or before August 15, 1994, to County's Health Services Director, or his designee, a written Final Activity Report which shall include, but not be limited to, an evaluation of the quantity, quality, and impact of the work undertaken in conducting the services provided under this Contract. 4. . Right of Inspection. Upon County's request, Contractor shall make available to County's AIDS Program Director or her designee for review, all records and materials relevant to the documentation of services provided under this Contract. In addition, as. requested by County, Contractor shall assist County's designated Contract Monitor in conducting site visits." 6. Meeting Requirements. The Service Plan of Standard Contract #22-435 is hereby modified by the addition of a new Paragraph 8. (Attendance at Meetings) to read as follows: "8. Attendance at Meetings. Contractor shall attend scheduled meetings and training sessions for Contractors of the Contra Costa HIV Interagency Network. In addition, Contractor shall regularly attend scheduled meetings of the Contra Costa HIV/AIDS Consortium." . 7. Modification of Subject. The Subject set forth in Paragraph 1.
