SUBJECT: patient Transfers
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
1. Authorize the Health Services Director or his designee to
approve the transfer of patients from County Health
Services Medical/Psychiatric facilities to other
locations, including facilities located outside of the
Unitecl..States, when it is in the best financial interest
of the County to do so.
2 . Authorize the Health Services Director or his designee to
arrange and approve payment for the transportation of
patients being transferred, including the wages and
travel costs of any County medical personnel that may be
required to accompany the patient.
II. FINANCIAL IMPACT:
Failure to approve this recommendation will result in an
increase in Hospital expenditures.
III. BACKGROUND:
During the normal course of business, the Department routinely
arranges for patients who no longer require acute
hospitalization to be transferred to a facility that provides
the lower level of care needed by the patient, (e.g. , a
rehabilitation hospital; a skilled nursing facility; the
patient's or relative's home) . While these transfers are
almost exclusively local in nature, periodically situations
arise where it is medically sound and financially neficial

to transport a patient one-way out of the United States. For
example, where relatives are able and ready to care for a
patient, but the relatives reside outside of the United States
and neither they nor the patient can afford the cost of
transporting the patient. Authorizing the Health Services
Director to transfer patients outside of the United States
would: (1) reduce unnecessary inpatient hospital utilization;
(2) allow the patient to be placed at a facility that provides
the most appropriate level of care for their specific illness;
and (3) allow the patient to be placed as close as possible to
their home and relatives.
