I. RECOMMENDED ACTION
APPROVE and AUTHORIZE the Community Services Director, or her
designee to execute a contract amendment with Sylvia Fisher
increasing the payment limit by $18,525 to a new payment limit of
$29, 175 for the provision of Mental Health Services to the Child
Development Program and - extend the term of the contract from
December 31, 1993 through June 30, 19,94.
II. FINANCIAL IMPACT
This contract is funded with Child Development and State Preschool
grant funds through the State Department of Education(DOE) . No

County funds are required.

III. CONSEQUENCES OF NEGATIVE ACTION
A decision hot to approve this Amendment could result in the loss
of needed Mental Health services to the Child Development and State
Preschool programs and severely impair the programs in meeting
State reporting guidelines.

IV. REASONS FOR RECOMMENDED ACTION
The Community Services Department, through this Amendment seeks to
extend the services for Mental Health provider for the full term of
the first State DOE fiscal year. There is an existing contract
between the Community Services Department and the Mental Health
provider. The original contract was in the amount of $10, 650 and
will pay the Mental Health provider until December 31, 1993. This
request seeks to continue her contract for its full one year term
which would end June 30, 1994.
