MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: November 9, 1993 SUBJECT: Approval of Contract Change Orders Nos. 8, 9, & 10 for the A.D. 1992-1; Mrack Road. project, Project No. 4500-6X5181-92 Specific Reques(s)or Recom mmdation(s)&Background&Justification I. Recommended Action: APPROVE Contract Change Orders Nos. 8, 9, and 10 in the approximate amounts of$14,000.00, $4,400.00, and$825.00, respectively, and AUTHORIZE the Public Works Director to execute them in order to compensate the contractor, Oliver de Silva, Inc., for work resulting from utility conflicts. 11. Financiallmpact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Assessment District 1992-1; Mrack Road. III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 10 exceeds $25,000.00, requiring Board approval. Significant utility conflicts became apparent only after the contract was awarded. These change orders were necessary to mitigate the changes in the scope of various items of work. IV. Consequences of Negative Action: The lack of approval would preclude payment for necessary work already completed.
