Please note that, for the FACT Committee, the appointments below are critical to the functioning of the Committee (due to the applicant's areas of expertise) irrespective of the eventual resolution of the HSAC/FACT Committee integration issue which is to be discussed by the IO Committee on September 28, 1992. I. Changes to FACT A. Resignations: 1. Sara Martin, representing HSAC, has resigned from FACT. No formal letter of resignation has been received to date. 2. Allan C. Moore, Central County At-Large Member, has resigned due to work and family commitments; his letter of resignation is enclosed. 3. Dianna Coggin, representing FACSAC,has resigned due to increased work responsibilities; her letter of resignation is attached. B. Appointments: — 1. Appoint David Joslin, member of FACSAC, to replace Dianna Coggin as FACSAC's representative to FACT. 2. Appoint Alice W. Furst to the vacant West County At-Large seat. Alice was interviewed by members of FACT and staff and is an extremely well- qualified applicant whose background with the East Bay Community Foundation will be invaluable as FACT begins a new RFP/proposal review cycle. Alice's resume and application are enclosed. 3. Appoint Michael Hall as the representative to FACT from the Child Abuse System Review Committee. Jennifer Summerton, Chair of the Committee, has notified me that Michael has been selected to -represent that To: Claude Van Marter Date: 8/24/92 Page: 2 Committee on FACT. I will forward his application to you as soon as it is received. C. Membership Category Change: Mary Lou Lucas, the current United Way representative to FACT, has recently resigned from United Way, but would like to remain a FACT Committee member. Mary Lou's past experience with the FACT RFP process is critical to ensure continuity for FACT, and the Committee requests her appointment to the West County At-Large seat vacated by Allan Moore. Staff will work with David Carrera to find a new United Way representative. II. Changes to FACSAC The following changes need to be made to the FACSAC Committee: District I: Mildred Davis has resigned, her resignation letter is enclosed. District IV: Lynda Sussman has resigned from the Committee; I will forward her letter of resignation if it is received. Dr. G.Bradley Gascoigne, appointed by the Board in May 1992, has informed me that the meeting date of FACSAC conflicts with a mandatory hospital meeting and he will not be able to accept his appointment to the Committee. Note: This leaves only one appointment from Supervisor McPeak's district, although Karen Mitchoff is working to remedy this. Gloria Tays has resigned from the Committee but no letter of resignation has been received despite repeated requests. I have enclosed the current FACSAC roster for your information; since there have been so many changes, I want to be sure our records are in agreement.
