Spooner, Contracts Administrator DATE: January 16, 1992 County Approval of Standard Agreement (Amendment) #29-396-3 with the State Department of SUBJECT!Health Services, Chronic Disease Branch - Tobacco Control Section SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chair to execute on behalf of the County, Standard Agreement (Amendment) #29-396-3 (State Number 89-97896 A-2) with the State Department of Health Services, Chronic Disease Branch - Tobacco Control Section, effective July 1, 1991, to amend Standard Agreement #29-396 (as amended by Standard Agreement Amendment #29- 396-2) . II. FINANCIAL IMPACT: Standard Agreement (Amendment) #29-396-2 resulted in State funds of $536,615 for County's Tobacco Control Project in FY 1989-90 from Proposition 99 Tobacco Tax revenues. Approval of Standard Agreement (Amendment) #29-396-3 incorporates the County's FY 1991-92 allocation; extends the term of the Contract through June 30, 1994; provides firm funding amounts through FY 1991-92 and projected funding amounts for FY 1992-93 and FY 1993-94, as follows: ENCUMBERED FUNDS PROJECTED FUNDING FY 1989-90 $ 536,615 FY 1992-93 $ 355,638 FY 1990-91 $ 533,729 FY 1993-94 $ 355.638 FY 1991-92 $ 355.638 TOTAL $ 711,276 TOTAL $1,425,982 TOTAL CONTRACT PAYMENT LIMIT $2,137,258 The final two fiscal year allocations are estimates that may increase, decrease or stay the same depending on fluctuations in future tobacco tax revenues. No County match is required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On June 19, 1990, the Board approved Standard Agreement #29-396 with the State Department of Health Services which allocated to the County, a share of Proposition 99 Tobacco Tax revenues for tobacco education services. Subsequent action (#29-396-1) resulted in approval of the Preliminary Tobacco Control Plan and Budget, and in $536,615 of State funds for FY 1989-90 (Standard Agreement [Amendment] #29-396-2) . Approval of Standard Agreement (Amendment) #29-396-3 continues the project through June 30, 1994, incorporates the County's FY 1991-92 allocation, provides firm funding amounts through FY 1991-92 and projected funding for FY 1992-93 and FY 1993-94. The Board Chair should sign ten copies of the agreement, nine of which should then be returned to the Contracts and Grants Unit for submission to e State.
