FISCAL IMPACT: The Request for Proposals specifies a $10, 000 application fee to cover County costs related to proposal review and site visits. The successful applicant will be required to enter into a Trauma Center Designation Agreement with the County which will require payment of an annual designation fee to the County to offset ongoing costs of trauma system monitoring and planning. The fee will be initially set at $115, 000. This is an increase of $15, 000 over the existing annual trauma center designation fee received by the County from John Muir Medical Center. SUMMARY: This RFP seeks proposals from Contra Costa County hospitals for designation of a single trauma center to serve all of Contra Costa County and establishes a review process to recommend selection to the Board of Supervisors. The RFP, selection process, and trauma center standards are similar to that established by the Board when the trauma system was initiated in 1986, except that "service to the community" has been added as an additional selection criterion. Under this criterion, selection will include consideration of specific programs or proposals in which the hospital is engaged or proposes to undertake if granted trauma center designation which reflect the hospital ' s overall commitment to the medical and health care of all segments of the County ' s population. Additionally, this RFP proposes to make trauma center designation permanent (except for cause) following two three-year contract periods. CLERK OF THE BOARD OF SUPERVISORS AND COUNTY ADMINISTRATOR AzaBY ,DEPUTY M382%1-83 -- BACKGROUND: In December 1985, the Board of Supervisors approved a Trauma System Plan for Contra Costa County. In accordance with State regulations and American College of Surgeons' guidelines on trauma center catchment areas, this plan specified one trauma center to serve the entire county. The plan was approved by the State Emergency Medical Services Authority. A competitive selection process was established by the Board and all hospitals invited to submit proposals for trauma center designation. John Muir Medical Center was the sole applicant. Following a rigorous proposal review and on site inspection by an outside trauma center review team, John Muir Medical Center was designated as the County' s trauma center on April 29, 1986, and a three year Trauma Center Designation Agreement was approved by the Board of Supervisors. This Trauma Center Designation Agreement was subsequently renewed for an additional three year period through April 28, 1992 . On June 21, 1991, the Health Services Department presented a report to the Board of Supervisors on the status of the County's Trauma Care System. In reviewing the past five years of operation, the Board of Supervisors found the County' s trauma system to be an unqualified success. At this time the recommendation for an RFP process is to allow other hospitals a final opportunity to seek trauma center designation by demonstrating their ability to provide trauma center services and to give all hospitals the opportunity demonstrate their commitment to medical and health care for all segments of the community.
