DIRECTOR T- s Costa SOCIAL SERVICE DEPARTMENT County DATE: DECEMBER 20, 19 91 ''rJ" x SUBJECT: APPROVE GRANT APPLICATION FOR SSI OUTREACH PROJECT SPECIFIC REYgT80RICOMTIONION(S)&BACKGROUND AND JUSTIFICATION Ratify the action ,;of the Social Service Department Director to apply for a grant from the Social Security Administration (SSA) for the SSI Outreach Project for a total of $249,520. The grant period is July 1, 1992 through June 30, 1993 . Furthermore, authorize the Social Service Director or his designee to accept the grant and to enter into an Agreement with the SSA, if the County' s grant proposal is approved. In addition, authorize the Social Service Director or his designee to negotiate and execute contracts for a portion of the services to be provided under this grant as follows: Agency Payment Limit Rubicon Programs, Inc. $137, 580 Hawkins Center 15,000 West County Senior, Services Network 15,000 Family & Community Services Agency 15,000 TOTAL $182, 580 FINANCIAL IMPACT The SSA grant will require a match in County- funds of $23,186. However, it is expected the Outreach Projec will suit in substantial cost savings in the County Ge al s ' stance and Homeless Shelter Programs. This program will increase outreach to populations who are not being reached by current efforts, such as frail elderly, homeless, and mentally incompetent persons. The SSA grant proposal has been submitted in an effort to fund this project for 12 months.
