SUBJECT: Approval of Contract Amendment Agreement #2'':6-183-2 with
Patricia Carini, R.N. , M.P.A.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair to execute on behalf of the County
Contract Amendment Agreement #26-183-2, effective May 31, 1991, to
amend Renewal Contract #26-183-1 (effective June 1, 1990 through
May 31, 1991) , with Patricia Carini, R.N. , M.P.A. , to extend the
term of the Contract to March 31, 1992 and to modify the Contract
Payment Provisions, with no increase in the Contract Payment
Limit.

II. FINANCIAL IMPACT:
Funding for this service has been included in, the Department's FY
1990-91 and FY 1991-92 Enterprise I Budget. The original payment
limit of the contract was $55, 000, and there has been no increase
in that amount of funding for this Contractor's services.

III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Patient Classification Systems are mandated " by JCAHO and Title
XXII, and an updated system for Merrithew Memorial Hospital is
essential because of the heavy workload for nurses and the level
of patient acuity.
At the conclusion of the 1988 contract negotiations between the
County and the California Nurses Association (CNA) , agreement was
reached with regard to the development of a new Patient
Classification System for Merrithew Memorial Hospital. Pursuant
to this agreement, a representative committee was convened, and
the committee subsequently selected Patricia A. Carini, R.N. ,
M.P.A. as the appropriate consultant to assist the Hospital with
selection and implementation of a new system':
On May 16, 1989, the Board approved Standard Contract #26-183 , and
on June 26, 1990, approved Renewal Contract #26-183-1 with Ms.
Carini. The project has taken longer to complete than originally
anticipated by all parties even though Ms. Carini has been working
steadily on the project since the County contracted with her in
1989. The termination date for Renewal Contract #26,183-1 was
May 31, 1991, and this inadvertently passed unnoticed by both the

take approximately six more months to complete the project and
therefore, the Department is requesting approval of Contract
Amendment Agreement #26-183-2 which will extend Renewal Contract
#26-183-1 to March 31, 1992. P
In addition to extending the term of the contract, Contract
Amendment Agreement #26-183-1 adjusts thea' Contract Payment
Provisions to appropriately reflect the payment amounts which have
already been paid to the Contractor and which will be paid to the
Contractor for the remainder of the project. The original
Contract Payment Limit for this project was $5E5, 000, and this has
not been increased over the term of the Project.
