MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: October 22, 1991 SUBJECT: APPROVE Project Contingency Fund increase of $38,000.00 and Contract Change Order No. 4 for the Yale/Cambridge Ave. Reconstruction Project, Project No. 0662-684100-90 Specific Request(s)or Recommendation(s)&Background&Justification I. Recommended Action: APPROVE Project Contingency Fund Increase of $38,000.00.and AUTHORIZE the Public Works Director.to execute Contract Change Order No. 4 in the approximate amount of $38,000.00 to compensate Bay Cities Paving & Grading for required extra work. II. Financial Impact: The project contingency funds are currently insufficient to cover these change order costs. The increase in contingency funds will be funded by Measure C-Half Cent Sales Tax (85%) and Senate Bill 300 State Transportation Partnership (16%). III. Reasons for Recommendations and Background: Additional curb, gutter, sidewalk and driveways were needed to complete the project. Because this change order. exceeds $25,000.00 Board Approval is required. IV. Consequences of Negative Action: The lack of APPROVAL would preclude payment to the contractor for required work and prevent the successful completion of the project.
