SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION
Designate December 1, 1991 through December 7, 1991, as California
Home Care Week.

III . BACKGROUND
A. National Home Care Week honors the dedicated service of the in-
home health and support service professionals who provide high
quality cost-effective care to patients in their homes.
During the week of December 1-7, 1991, the Contra Costa County
Home Health Agency will join with numerous health care
organizations throughout the state in observing National Home
Care Week.
B. Established in 1968 as an adjunct program in Public Health, the
Contra Costa County Home Health Agency became an independent
program in the Health Services Department, Public Health
Division in July, 1990. As an important part of Contra Costa
County's continuum of care system, the Home Health Agency
strives toward maintaining patient independence through acute
care and restorative services that significantly reduce the need
for institutionalization and re-hospitalizations.
C. On a daily basis, the Home Health Agency's professional team of
nurses, physical, occupational, and speech therapists, medical
social workers and home health aides take their special skills
into patients' homes throughout the County. High risk newborns
to the debilitated elderly, acutely ill to the terminally ill
are treated with dignity and respect as the team addresses the
medical, social, emotional, and environmental needs of patients
and their families. Approximately 22,000 - 24,000 visits are
delivered to Contra Costa County residents each year.
D. The Home Health Agency prides itself on working cooperatively
and collaboratively with other County-based programs, as well as
community-based organizations. Through these efforts,
constructive planning occurs for longer term supervision, once
the short-term intensive work of the Home Health Agency has been
completed.
IV. FUTURE
The Home Health Agency is very clear on its mission as an integral
part of the Health Service Department's continuum of care. With home
health care proving to be the health care trend of the future for
meeting the needs of an increasingly high risk population, across all
age groups, a rapidly aging population, and because of its cost
containment features, the Contra Costa County Home Health Agency is
solidly in place and committed to meeting the County's future
demands.
