Spooner, Contracts Administrato t I DATE: November 9, 1990 (0 County SUBJECT: Approval of Medical Specialist Contract #26-853-11 with Tara Hojiwala, M.D. SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Health Services Director, or his designee, (Frank Puglisi, Jr. ) to execute on behalf of the County, Medical Specialist Contract #26-853-11 with Tara Hojiwala, M.D. , (specialty: Anesthesiology) for the period November 1, 1990 through October 31, 1991 to be paid as follows: a. For consultation and training, $42 .80 per hour; b. $28.00 per RVS Unit for each medical procedure; C. In addition, for on-call services: (1) 500 per weekend on-call duty period (2) 150 per holiday on-call duty period (3) 50 per weekday evening on-call duty period (4) 100 per weekday on-call duty period II. FINANCIAL IMPACT: Cost to the County depends upon utilization. As appropriate, patients and/or third party payers will be billed for services. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: For a number of years the County has contracted with Medical and Dental Specialists to provide specialized professional services which are not otherwise available in its hospital and clinics. Under Contract #26-853-11, Tara Hojiwala, M.D. will continue to provide anesthesiology services for Merrithew Memorial Hospital and Clinics through October 31, 1991. This Medical Specialist Contract is prepared in the standard format approved by County Counsel ' s Office.
