SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-834-12 with Respiratory Medical Group, Inc.
(specialty: Internal Medicine) for the period November 1, 1990
through October 31, 1991 to be paid as follows:
a. For consultation and training, $42 . 80 per hour, and
b. For surgery, fifty percent (500 of the fee stated in the
official fee schedule approved by the Division of Industrial
Accidents, State of California, in effect on the date of
surgery.
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics. Under
Contract #26-834-12, Respiratory Medical Group, Inc. will continue to
provide Internal Medicine Services for Merrithew Memorial, Hospital
and Clinics through October 31, 1991.
This Medical Specialist Contract is prepared in the standard format
approved by County Counsel 's Office. ,

I. RECOMMENDED ACTION:
Approve and authorize the Health services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-835-12 with Burton Baker, M.D. (specialty:
Vascular and General Surgery) for the period November 1, 1990 through
October 31, 1991 to be paid as follows:
a. For consultation and training, $42.80 per hour, and
b. For surgery, fifty Percent (50%) of the fee stated in the
official fee schedule approved by the Division of Industrial
Accidents, State of California, in effect on the date of
surgery.
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics. Under
Contract #26-835-12, Burton Baker, M.D. will continue to provide
Vascular and General Surgery Services for Merrithew Memorial Hospital
and Clinics through October 31, 1991.

SUBJECT•'Approval of Medical Specialist Contract #26-866-8 with
Mark A. Silvert, M.D.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Medical
Specialist Contract #26-866-8 with Mark A. Silvert, M.D. (specialty:
Urology) for the period November 1, 1990 through October 31, 1991 to
be paid as follows:
a. For consultation and training, $42 .80 per hour, and
b. For surgery, fifty percent (50%) of the fee stated in the
official fee schedule approved by the Division of Industrial
Accidents, State of California, in effect on the date of
surgery.
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics. Under
Contract #26-866-8, Mark A. Silvert, M.D. , will continue to provide
urology services for Merrithew Memorial Hospital and Clinics through
October 31, 1991.
This Medical Specialist Contract is prepared in the standard format
approved by County Counsel 's Office.
