SUBJECT: Approve submission of Funding Application #29-391 to the State
Department of Health Services for the AIDS Medi-Cal Waiver Program
SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair to execute on behalf of the County, Funding
Application #29-391 for submission to the State Department of Health Services
(Office of AIDS) for an exemption under the AIDS Medi-Cal Waiver Program, for
the period April 1, 1990 through December 31, 1991, to allow the Health Services
Department Home Health Agency to provide direct home health care services to AIDS
Medi-Cal Waiver Program clients.
II. FINANCIAL IMPACT:
Approval of this application by the State will allow the Department's Home Health
Agency to provide direct home health care services to AIDS Medi-Cal Waiver Program
clients. Payment is provided for specific services at established Medi-Cal rates.
The total funded amount will be determined by the number of "slots" awarded and
services provided.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Current staff of the Public Health Division AIDS Program is experienced in
providing case management services for people with HIV Disease in Contra Costa
County. The AIDS Program currently provides case managed care to persons with
ARC and AIDS through the State-funded Pilot Care Project. The AIDS Medi-Cal
Waiver Program goals are to lessen the financial cost of care for people with
AIDS and ARC, which are historically driven by hospitalizations and other
institution-based care, and to provide the most humane and appropriate levels of
care in the most appropriate setting for the client. Participation in the program
will allow the AIDS Program to offer case managed home and community-based care
to a greater number of clients in the county.
The nurse case manager of the AIDS Program will determine a client's eligibility
for the program. If found eligible, the case manager, along with the client and
,other providers of care, will develop a care plan that will be regularly assessed
and updated. Services which will be provided by contract agencies will include
skilled nursing care by a registered nurse and licensed vocational nurse,
psychosocial couns'eling, attendant care, homemaker services, and minor physical
adaptations to the home. The case manager will work closely with California
Children's Services and the Maternal, Child and Adolescent Program to assure the
Medi-Cal supplement for foster parents of eligible children.
In order to meet the deadline for submission, a draft copy of the application
has been forwarded to the State, but subject to Board approval. The Board Chair
should sign three copies of the application, two of which should t be returned
to the Contracts and Grants Unit for submission to the State.
