CONTRA COSTA COUNTY CLASSIFICATION QUESTIONNAIRE Public Health Nurse CIIDP/P1IN Case ManagerCHDP NAME DEPARTMENT DIVISION OFFICIAL JOB CLASSIFICATION WORKING I JOB TITLE -FL-ACEOF WORK AND ASSIGNED HOURS Time Required Description of Work: A. CHDP PM 160- Follow-U]2 & Case Management Activities 1. Review CHDP PM 160s, initiate appropriate follow- up and case management activities to assure Dx Tx services are received. 60% 2. coordinate activities with providers and other community agencies involved in services to assure that total health needs are being met. 3 . Make home visit if necessary to interpret and facilitate follow-up to Dx and Tx. 4 . * Assist in data collection activities. Maintain data, assure that data collection kept up-to-date. 5. Participate in evaluation activity with Program Director, Deputy Director and other professional staff. 6. Assist in development of case management procedures with Deputy Director and EPSDT Coordinator. 7 . Assist in development of forms, letters and other case management documents. Program Activities A. Attend meetings with S.S.D. State personnel and other community agencies to confer, plan and discuss CHDP target population issues and concerns. 10% B. Represent program at various community activities in advocacy role for the CHDP population. C. Provide reports to the MCAH Board when assigned. Provider Relations A. Participate in outreach and follow-up services with community medical resources; e.g. , hospital 5% clinic, private providers and community agencies. Communicate with CHDP providers to provide programmatic information related to CHDP eligible children as well as consult and problem solve on case management. IV. Health Education and Training A. Work with the Health Education staff to provide anti tobacco use education activities for the target population. 20% B. Participate in health education activities including anti tobacco use in settings such as clinics, schools, special education, parenting clinics, etc. C. Assist in training staff who may assist in health education and outreach activities as these pertain to outreach to the target population. V. Other Activities 5% A. Assist in audits and perform quality assurance activities in provider offices. B. Assist in other CHDP Activities as assigned. C.
