RECOMMENDATION AUTHORIZE the Director of General Services to EXECUTE, on behalf of the County, a Lease with Maria Casazza and Rina Casazza for the premises at 2923 MacDonald Avenue, Richmond, for occupancy by the Health Services Department, under the terms and conditions more particularly set forth in said Lease. DETERMINE that the above project is a Class 1 Categorical Exemption under the California Environmental Quality Act. DIRECT the Community Development Director to FILE a Notice of Exemption with the County Clerk. II. FINANCIAL IMPACT Payments required are to come from the budget of the Health Services Department. III. REASONS FOR RECOMMENDATIONS/BACKGROUND This project will provide a clinic for older adults in the west county area. These patients are now seen in the central county clinic facility. Gilbert, Director of General Services By: Chris' eardsley Kramer, Lease Management Division SUBJECT: CORRECTION OF BOARD ORDER NO. 1-045 It is necessary to make a correction of the Lessor's name on the Board Order for the new Lease for 2923 MacDonald Avenue, Richmond. The Lease is with the owners of the building, Maria Casazza and Rina Casazza, who also signed the Lease. On County Counsel 's advice, it is not necessary to have the Board approve this change. I am sending you a corrected Board Order, along with your fully executed copy of the Lease. Please have the Board Order signed and dated, and send me a copy for distrubution. Thank you. Cisterman Director of Personnel July 17, 1989 Henry Clarke, General Manager Contra Costa County Employees Assn. , Local No. 1 PO Box 222 Martinez CA 94553 Dear Mr. Clarke: This is to confirm our agreement regarding the temporary overtime settlement for all persons employed as Deputy Public Defender I as follows: All persons employed as a temporary Deputy Public Defender I in the Contra Costa County Public Defenders' Office during the period of November 15, 1981 to the date this side letter is signed, are entitled to be paid $60 a month for each month of employment as a temporary Deputy Public Defender I during the above stated period. The Office of the 'Public Defender has prepared, based on employment records, a list of all persons entitled to this payment, the time periods covered, and the amounts of each payment to each eligible person. A copy of this list is attached to this side letter. .The County will send by certified mail to the last known address of eacli person on the attached list, a form to sign and return to the County within 6 months from the date of mailing. The form will indicate the amount of money each per- son is entitled to receive and inform the recipient that their signature on the form indicates their acceptance of the County's offer and a waiver of their right to pursue any and all other avenues of restitution. Upon receipt of each signed form, the County Auditor-Controller will processthe applicable payment to the address indicated on the form by the recipient.
