SUBJECT: STATUS REPORT ON THREE-YEAR AIDS RESPONSE PLAN
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS
1. Acknowledge receipt of the attached reports from the Health
Services Director outlining the organization, structure and
budget for the County' s AIDS Program and outlining the
presentations which have been made and are scheduled for
Kaiser-Permanente ' s play "Secrets" .
2. Request the County Administrator to give priority to
processing the Health Services Department ' s request for two
additional Health Educator Specialists to develop AIDS
prevention • programs in ethnic minority communities
throughout the County where the incidence of HIV infection
and AIDS cases, relative to population size, is rising. One
of these positions will be bilingual Spanish-speaking and
the other position will target Black communities.
3 . Authorize the Health Services Director to establish a
department-level AIDS Advisory Committee to review the
County' s plan for the AIDS education and prevention money
which will be subvented to the County in a block grant.
4. Remove this item as a referral to our Committee.
BACKGROUND:
On September 19 , 1989 the Board of Supervisors approved a report
from our Committee which- requested that the Health Services
Department provide our Committee with several pieces of
information. The Department was also asked to schedule
performances of "Secrets" at schools throughout the County.

On December 11 , 1989 our Committee met with Wendel Brunner, M.D. ,
Director of Public Health and Francine Wise, R.N. , from the
Health Services Department. Dr. Brunner and Ms. Wise reviewed
the attached reports with our Committee which provide all of the
information which we had requested in September.
The State has adopted a program under which all of the AIDS
education and prevention funds coming to any agency in this
County will be block-granted through the County to both County
and private non-profit agencies . This subvention program
requires that an AIDS Advisory Committee be established in order
to provide input to the County' s plans for distribution of these
funds. We believe that this Advisory Committee is best left as a
department-level Committee rather than asking the Board of
Supervisors to establish yet another Advisory Committee.
It seems clear that the Health Services Department is doing an
outstanding job of attempting to cope with the AIDS epidemic in
Contra Costa County and we commend their work, as is reflected in
the attached reports.

Subject: Presentations of "Secrets" in Contra Costa Countv
We are delighted to learn that the Board shares the enthusiasm of the Health
Services Department for Kaiser-Permanente's play, "Secrets." In response to the request
that the Health Services Department arrange for a showing of the play in Contra Costa
County, the AIDS Program staff contacted Kaiser, the County Office of Education, school
districts and consulted with the TeenAge Program within the Public Health Division.
To date, "Secrets" has been shown at many Contra Costa County high schools,
including Miramonte, Campolinda, Las Lomas, College Park, Mount Diablo, Northgate,
Olympic, Pittsburg, Marina, and Riverside. Additionally it has been presented at Pride
House and Lion's Gate, and arrangements have been made for presentations at Acalanes
and John Swett High Schools.
In cooperation with the County Office of Education and the Juvenile Probation
Department, the Health Services Department has arranged to present "Secrets" at Juvenile
Hall on December 18 at 1:30 p.m. and at Byron Boys' Ranch on December 19 at 10:30 a.m.
Interested individuals are invited to see the presentations at Juvenile Hall and Byron Boys'
Ranch.
AIDS Program education staff will attend the next meeting of the County Office of
Education meeting of Assistant Superintendents and Curriculum Directors. At that time
we will show a promotional video provided by Kaiser-Permanente, distribute information
on how to schedule a performance and encourage school districts to arrange a presentation
in their schools.
As we all know, an epidemic as deadly and as complex`as AIDS requires that we all
use every ounce of creativity, every educational tool we have, and all .the diversity of
approaches we can imagine to reach everyone at a very personal level. "Secrets"represents
one of those creative, new ways to impress upon young people the reality of AIDS. Thank
you for letting us know that you share our enthusiasm for it.

Organizational and Financial Profile
Contra Costa County's Health-Services Department has organized its AIDS Program
to respond to the epidemic of HIV Disease in our county. The AIDS Program,, a
Communicable Disease Control Program unit within the Public Health Division,. works
within our county's network of health providers, with schools, private industry and
community-based organizations to coordinate an effective community response to this
deadly epidemic.
I. AIDS Program Organization
The AIDS Program hired its first full-time employee whose. time was totally
dedicated to AIDS work in April 1986. In October 1989 there are twelve permanent, full-
time employees, five full-time contract workers, and ten part-time contract workers.in the
AIDS Program. Each of these is assigned to one of four work teams: Education. and
Prevention,Epidemiology and Testing, Client Services, and the Administrative Team.' Each
of these team leaders as well as.three members of. the Administrative Team. form a
Management Team. An organizational chart and staffing summary follow on pages three
and four of this report. Position descriptions begin on page six.

The AIDS Program has grown rapidly over the past two and one-half years into
a public health program with a budget of more than $1 million. This growth has resulted
in complex administrative requirements in planning, program development, fiscal
management, personnel and contracts management, and information systems management.
The Administrative Team consists of two full-time permanent public health program
specialist positions, one full-time permanent senior clerk and one full-time contractor.
The Education and Prevention Team has responsibility for promoting healthy
behaviors for the prevention of HIV Disease. This team is available to groups for
presentations and also coordinates the activities of other State-funded AIDS education and
prevention contractors to assure maximum coverage of targeted groups and to minimize
duplication of effort throughout the county. The team subcontracts with the Contra Costa
County AIDS Task Force which targets workplace education and men who have sex with
men, and with the TeenAge Program of Public Health's Maternal, Child and Adolescent
Health unit which targets high-risk adolescents. This team also targets individuals with
chemical dependency by providing street outreach and education on the streets of cities in
our county. This part of the team also provides education to clients at substance abuse
programs in the county. This team currently consists of a public health epidemiologist, a
senior level health educator, an experienced level clerk and three full-time contractors.
The Epidemiology and Testing Team is charged with discouraging the spread of
AIDS by counseling individuals at risk and testing for HIV infection. They also monitor
the incidence of AIDS, AIDS Related Conditions and HIV seroprevalence in Contra Costa
County. This team currently consists of a full-time contractor as coordinator of the team,
a full-time permanent public health epidemiologist, a full-time permanent experienced-
level clerk and ten part-time contractors.
The Client Services Team promotes and organizes treatment and supportive services
for people with AIDS and HIV infection, their partners, families and friends. A full-time
permanent public health program specialist supervises this team. In addition, the team is
comprised of a full-time permanent public health nurse, a full-time permanent medical
social worker, and a full-time permanent experienced level clerk.

II. Staffing summary
1. Twenty-seven individuals are currently working.for the Contra Costa County
Health Services Department's AIDS Program.
2. Positions:

3. Of the twelve employees, all are permanent.
.4. A regional breakdown of services in the county is provided on the next page
of this report. All AIDS Program services are offered to all county residents.
The three Community Health Outreach Workers (full-time contractors)work
primarily in East and West County. The part-time contractors work primarily
at Alternative Test Sites and Sexually Transmitted Disease Clinics in Concord,
Richmond and Pittsburg.
III.. AIDS Services by Region of County
While the AIDS Program's central office is in Martinez, services are provided
throughout Contra Costa County. Alternative Test Sites,where individuals receive free and
anonymous HIV testing and one-on-one counseling about AIDS risk reduction practices,are
in Richmond, Concord and Pittsburg. Community Health Outreach Workers (CHOWS)
work with one-on-one AIDS prevention education to clients with chemical dependency
primarily in East and West Contra Costa County. The following page lists AIDS Program
services by region of the county.

IV. Position descriptions
A. Administration
Rusty Keilch is the AIDS Program Manager and coordinator of both the
Administrative team and the Management Team. Rusty's is currently a program
specialist I position. She has overall responsibility for the AIDS Program's
administration and has direct supervision of all members of the Administrative Team
and other team leaders and provides policy and operational direction to AIDS
Program staff. Rusty prepares annual budgets for the AIDS Program for the
Communicable Disease Program Director and coordinates staff development and
staff and program evaluation. She coordinates service,staffing, and budget proposals
for funding applications. Rusty works with the Communicable Disease Control
Program Director and the Public Health Division Director to develop strategies and
policies for AIDS Program activities and represents the County at regional and State
wide AIDS organizations and coordinating groups.
Nancy Warren, Assistant AIDS Program Manager, became a full-time, permanent
employee program specialist I for the county in May of this year after serving as a
full-time contract employee for 19 months. As Assistant Manager of the Program
she assists the Program Manager in the areas defined above. Nancy has primary
responsibility for planning, program development, policy analysis and proposal
writing.
Marlina Hartley is a full-time permanent employee and works for the AIDS Program
as a senior clerk. She currently supervises all the AIDS Program support staff and
assist in managing personal services contracts and subcontracts with agencies
providing care to AIDS patients.
Jered Gross is a full-time contractor for the AIDS Program. His work includes
computer systems development,computer technical support,fiscal data management,
analysis and reporting.

B. Education and Prevention
Dale Jensen,the supervisor and coordinator of the Education and Prevention Team,
is a permanent full-time county employee who is currently classified as a public
health epidemiologist. Dale oversees literature distributions, public speaking
commitments, work with employers to develop AIDS/HIV education in the
workplace, work with schools, targeting AIDS education to women of childbearing
age, work in the jails and substance abuse programs and coordinates prevention
education activities with other institutions, agencies and community-based
organizations in the county. Dale is staff to a network of AIDS service providers in
West County, the West County AIDS Network (WeCAN). Dale also supervises the
street outreach component of the Education and Prevention Team.
Lisa Kleppel is a senior level health educator in the Education and Prevention team.
She is a permanent full-time employee. Lisa serves as staff to a network of service
providers to the Latino community, El SIDA. She is also the contract monitor of
the two education and prevention subcontractors, the Contra Costa County AIDS
Task Force and the TeenAge Program. Lisa also has primary responsibility for
dealing with schools in the county, assisting them in developing and implementing
AIDS curricula in the schools.
Vincent Bell, Jose Peters, and Gilbert Soberal are full-time contractors in the
Education and Prevention team who collectively make up the street outreach team.
As Community Health Outreach Workers (CHOWs), they work primarily in West
and East County, providing both individual HIV risk reduction education to current
users of intravenous drugs on the streets of cities in East and West county and to
groups of clients in recovery programs throughout the county. These contractors
have each worked full-time as outreach workers with the AIDS Program for one and
a half to two years. In the current fiscal year, positions will be created classified as
Public Health Aides.
James Benham is a full-time permanent experienced-level clerk who assists in the
ordering of supplies and health education materials. Jim provides general clerical
support to the AIDS Program.
Proposed Health Educator Specialists. The creation of two positions has been
proposed by the Health Services Department. These positions will develop AIDS
prevention education programs in ethnic minority communities throughout the
County where the incidence of HIV infection and AIDS cases, relative to population
size, is rising. One position has been requested as bilingual Spanish-speaking to
address the needs in the Latino communities and the other position would target
Black communities.

C. Epidemiology and Testing
Juan Reardon is a full-time contractor and coordinator of the Epidemiology and
Testing team. Juan has oversight of all testing programs and all surveillance
activities for the AIDS Program. He oversees the provision of HN testing and
counseling services throughout the county and the maintenance of active surveillance
of AIDS cases. Juan oversees the State-funded Alternative Test Sites in Concord,
Richmond and Pittsburg where testing is free and anonymous. He oversees testing
activities at Public Health Division Sexually Transmitted Disease clinics and offers
assistance to other providers in the county who offer HIV testing; i.e., BAART
methadone clinics and Planned Parenthood clinics. Juan also coordinates HIV
testing and counseling services to victims of certain crimes which might put them at
risk for HIV infection and to health care workers who may have been occupationally
exposed to HIV. Juan coordinates a CDC-funded seroprevalence study at selected
clinics throughout the county and monitors epidemiologic surveillance to maintain
the State mandated AIDS case registry.
Denise Johnson, a permanent full time county employee in the classification of
Public Health epidemiologist, is responsible for AIDS case reporting. Denise
conducts active surveillance activities at Merrithew Memorial Hospital and has
frequent contact with key personnel at the ten other hospitals in the county to
encourage AIDS case reporting. Denise provides information to physicians in the
county to encourage case reporting. Denise also provides contact follow up on
persons who have been named as having been potentially exposed to HIV.
Lavelle Anderson, a permanent full-time county employee as an experienced level
clerk provides support services for the Epidemiology and Testing team. Lavelle is
responsible for all data entry of testing data from Alternative Test Sites, STD clinics,
primary care clinics and at Merrithew Memorial Hospital. She also provides data
entry for the CDC-funded seroprevalence study.
Ten HIV counselors and phlebotomists work as part-time contractors with the
Epidemiology and Testing team at Alternative Test Sites and STD clinics. These
contractors currently equal 2.5 full-time equivalents.

D. Client Services
Jack Champlin is a full-time permanent county employee in a public health program
specialist position and supervises the Client Services team. Jack oversees the State-
funded Pilot Care Case Management Program which provides direct services for
people with AIDS and ARC, coordinates services for clients and is a mechanism to
collect and analyze data on costs of care. Jack oversees the maintenance of an
inventory of medical, mental health, and social services resources for persons with
HIV Disease. Jack coordinates work with public and private health care providers
to identify services available for persons with HIV disease and to identify gaps in
services available. Jack also coordinates education and training programs for health
care professionals in an effort to increase the number of care providers willing to
treat HN infected persons and to improve the quality of care provided.
Jane Holcomb is a full-time permanent county employee and is the public health
nurse case manager of the Pilot Care Case Management Program. In FY 88-89,
Jane provided case management services to 77 persons with AIDS or ARC which
included 71 cases receiving home health care services or licensed nursing supervision.
Susan Freeman is the medical social worker on the case management team. works
Susan provides social work services for clients on the case management roster and
to their family members, partners and friends and offers assistance to discharge
planners and other providers of care at hospitals throughout the county.
Mary Parish is a part-time contractor working with the Client Services team in
several capacities. Mary coordinates the volunteer-led support groups which are
available to people with HIV infection, partners of people who are HIV infected,
parents of people with HIV infection and a grief group for people who have lost a
loved one due to HIV Disease. Mary also leads the federally-funded AIDS
Education and Training Center program which provides education and training to
health care professionals throughout the county including physicians,nurses, dentists,
dental auxiliary workers and mental health professionals. Mary also works as liaison
with county mental health programs.
Ann Loflin is a permanent full-time experienced level clerk who provides support
for the Client Services team. Ann is responsible for data entry for the Pilot Care
Case Management Project and maintenance of the Resource Directory utilized by
each of the AIDS Program teams.

It has been known for some time that AIDS, like a host of

The disproportional trend is becoming increasingly The4otal number of diagnosed AIDS cases Ir ..
apparent in Contra Costa County. In 1986, the CoM�Costa County�s now 344
proportional ratio of Black to White cases was about equal'
(1:1). In only two years, by 1988, the ratio was over 3:1.
In 1987 and 1988,the proportional ratio of Latino to White
cases was also about equal (1:1), but in the first three _ Black and Latino communities. This factor alone,
quarters of 1989,there has been a substantial increase in however,does not explain the disproportion. Even among
Latino cases in the county producing a proportional ratio intravenous drug users being tested, higher percentages
of approximately 2:1 when compared to Whites. of Blacks and Latinos are found to be seropositive.
Although these numbers are subject to random fluctuation
and are influenced by lack of or delays in reporting and/or
diagnosis, they nonetheless reflect the national trend. AIDS Cases per 10,000 County Residents
