SUBJECT; Approval of Contract Amendment Agreement 424-301-14 with Center
for Human Development to extend the School-Community Primary
Prevention Proeram
SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION :
Approve and authorize the Chair to execute on behalf of the
County, Contract Amendment Agreement 424-301-14 with Center for
Human Development , effective October 30, 1987 , to extend the
10-month Contract 424-301-13 for six additional months through
April 30, 1988 and to increase the previous 10-month contract
payment limit of $52 , 511 by an additional $31 , 506 for a new
total contract payment limit of $84 , 017' for the 16-month period
from January 1 , 1987 through April 30, 1988 .
II . FINANCIAL IMPACT:.
This program continues to be 100% funded by State funds
available through ' SB 110 (formerly SB 1409) through a contract
with the State Department of Alcohol and Drug Programs .
III . REASONS FOR RECOMMENDATIONS/BACKGROUND :
On ' March 319 1987 , the Board approved Novation Contract
#'24-301-13 with Center for Human Development for continuation of
the School-Community Primary Prevention Program through
October 31 , 1987 .
By letter dated September 8 , 1987 , (copy attached) the State has
informed the County that the School-Community Primary Prevention
Program has been scheduled for renewal by the Department of
Alcohol and Drug Programs for the twelve-month period ' from
November 1 , 1987 through October 31 , 1988 . The County contracts
with the Center for Human Development to carry out the program
activities and services . To allow for continuation of program
services while waiting for the State to process the pending
contract documents , it is necessary to extend Contract
424-301-13 with the Center for Human Development .
This document has been approved by the Department ' s Contracts
and Grants Administrator in accordance with the guidelines
approved by the Board ' s Order - of December 1 , 1981 (Guidelines
for contract preparation and processing, . Health Services
Department) .

Contract Number: D-0026-6 Renewal for School-Community Primary
Prevention Program
This is to inform you that your contract with the Department of Alcohol and
Drug Programs for the School Community Primary Prevention Program is
scheduled for renewal. Renewal will be dependent upon satisfactory
performance of the contract specifications, submission of a satisfactory
renewal plan and availability of funds.
The Department intends to allocate $61,650 to contract for services from
your county for a twelve (12) month period from
November 1, 1987 through October 31, 1988.
The following items comprise your renewal plan and must be submitted in
order for your county to be considered for continued funding:
1. Program Design: Briefly describe the overall prevention program
design currently used by the program. Include specifics regarding
the work being done to meet identified:needs (e.g. .curriculums,
forums, provision of technical assistance to community, etc.) If the
program design reflects a departure from your original proposal
submission you must provide an explanation for the departure and a•
justification and rationale for any change in the program design.
2. Target Population: Describe the target population, the designated
geographical service area and the anticipated number of participants
that will be served by the program during the contract period. You
must further detail the proportion of service utilization of program
recipients by ethnicity (e.g. 3211, Native American; 8% Caucasian;
3% Aleutian). The Department is particularly concerned about the
amount of public resources directed to the needs of minority groups.
3. Workscope/scheduling: Utilizing the attached forms, (attachments A &
B) provide a detailed workplan -for the renewal period.
This is to include 1) clear statements of broad program goals, 2)
measurable objectives relating to each goal, 3) the specific activities and
tasks that will be performed to meet each objective, 4) the expected
outcomes or intended effects of each activity and 5) timelines and dates of
key deliverables.
4. Budget: Utilizing the attached form, (attachment C) provide a
detailed line item budget that reflects the total contract award. All
line items must be fully explained and justified on a separate
attachment with the budget as backup documentation for estimated
costs.
Personal Services are to include each authorized position listed-by title,
and a line item for fringe benefits showing the percentage rate. A line
item for each authorized position must provide the monthly salary range,
number of months the position will be filled and the percent of time the
position will devote to the project. A salary range should be shown to .
permit any anticipated wage increases during the period covered by the
contract. All salaries must be fully justified in your attachment.
Operating Expenses are to include all other direct cost line items, such as
rent, travel and per diem, supplies, insurance, consultants, etc. A line
item for rent must identify the number of square feet to be utilized for
the project and the cost per square foot. A line item for equipment rental
must identify the type of rental equipment. Travel and per diem rates may
not exceed state rates. Consultant costs must be fully explained and
justified,. including cost per hour and number of hours. ' Fees should be
consistent with the annual income of each consultant and market rates for
similar services.
There should be no line items entitled miscellaneous or other. All line
items must be fully explained and justified.
Additionally, the program must obtain a 10*4 match from the school(s) or
school district(s). For this contract period, at least 5,% match shall be
an actual cash contribution and the other half shall be an in-kind.
. 5. Other: Your contract renewal submission must be accompanied by a
cover letter signed by the County Office of Education and the County
Drug Abuse Program Office indicating both have reviewed and approved
the contents. The contract renewal package must be received by this
office no later than October I, 1987. There will be insufficient time
to process the contract after the date without disruptions in service
delivery.
