Spooner, Contracts AdministratorCosta DATE: December 31, 1986 C(Xj "1 SUBJECT: Approval of Standard Agreement (Amendment) X629-217-22 with the State Department of Health Services (State X686-89178, A-1) for the Sexually Transmitted Disease Control Program SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Standard Agreement (Amendment) #29-217-22 (State 6686-89178, A-1) with the State Department of Health Services, effective December 31, 1986, to amend Standard Agreement X686-89178 (effective July 1, 1986 through December 31, 1986) to extend the Sexually Transmitted Disease Control Program through June 30, 1987 with a $20,781 increase in the contract payment limit for a total of $41,562 for the fiscal year term. II. FINANCIAL IMPACT: Approval of this amendment by the State will result in an increase of $20,781 in State funding for this program, from $20,781 to a new total of $41,562 for the fiscal year term. Sources of funding for this program are as follows: $41,562 State Department of. Health Services 15,798 County In-Kind Share in FY 1986-87 Budget . .7,170 County Fixed Overhead $64,530 Total Program III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On October 7, 1986 the Board approved Contract #29-217-21 with the State Department of Health Services for continuation of the long standing Sexually . Transmitted Disease Control Program through December 31, 1986. The attached State-prepared amendment continues State funding for this program through June 30, 1987. The goal of the program is to reduce the transmission of gonorrhea and other sexually transmitted diseases through detection of asymtomatic disease among susceptible female and selected male population who are at high risk of being infected. This program interacts with family planning and other sexual education programs. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 1, 1981 (Guidelines for contract preparation and processing, Health Services Department) . The Board Chairman should sign eight copies of the amendment, seven of which should then be returned to the Contracts and Grants Unit for submission to the State Department of Health Services .
