Spooner, Contracts Administrator �a DATE: January 28, 1987 @ CO^ SUBJECT: Approval of Novation Contract #24-337-3 with Telecare Corporation SPECIFIC REQUEST(S) OR RECOMMENDATION(S ) & BACKGROUND -AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chair to execute on behalf of the County, Novation Contract #24-337-3 with Telecare Corporation in the amount of $45,996 for the period July 1, 1986 - June 30, 1987 for provision of alternative care placement service for Contra Costa County geriatric patients. This document includes a six- month automatic extension from June 30, 1987 through December 31, 1987 in the amount of $22,998. II. FINANCIAL IMPACT: This contract is totally funded with County funds, representing the County's match to State Short-Doyle/Mental Health funds that are expended by the County of Alameda to provide mental health services for Contra Costa County residents as part of the County's Regional 302f Buy-Out Program. The contract payment limit is fully funded in the Department's FY 1986-87 Budget (Cost Center 5942). III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On December 9, 1986 the Board approved Contract Amendment Agreement #24-337-2 to extend, through February 28, 1987, Telecare Corporation's contract for alternative care placement . service for Contra Costa County geriatric patients . Novation Contract #24-337-3 replaces the prior contract extension and continues services through June 30', 1987, with an automatic extension through December 31, 1987. This contract provides placement services to make necessary alternative care options available for elderly patients at Garfield Geropsychiatric Hospital who have physi- cally and/or_ mentally deteriorated to the point where they no longer benefit from the intensive rehabilitation program offered at Garfield. The alternative care options offer a secure, protected environment for our patients who otherwise would not be accepted into skilled nursing facilites due to combined medical and psychiatric problems. This document has been approved by the Department's Contracts and Grants Administrator in accordance with the guidelines approved by the Board's Order of December 10 1981 (Guidelines for contract preparation and processing, Health Services Department) .
