40/40 Under -the supervision of the CHDP Deputy Director the following activi-
ties will be carried out:
I. CASE MANAGEMENT ACTIVITIES
50% A. _Foster Care Social Service Referral 1135711 form and case management.
1. Review "35711 referrals received from County Dept. of Social
Services on all children placed in foster care initially or
on change of placement. Assess for appropriate staff (EPSDT
workers or PHN) to provide contact.
2. Make home visits as necessary to assist foster parents in
meeting mandates for preventive health care by assisting with
support services.
3. Assist in the operational implementation of the foster care
component.
4. Assist in data collection activities.
'i: Assist in the development of forms, letters and other case
management documents. -
6. Tract all foster care children to assure entry into a system
of ongoing comprehensive medical and dental care.
7. Provide consultation and technical assistance to foster care
parents, social workers, EPSDT staff and public Health
Nursing.
8. Serve as CHDP/EPSDT linkage to the medical com unit.y_in cases
related to foster care medical en a concerns/issues.
9. Develop, maintain and monitor "in-house" system of case
management to assure quality of services.
10. Work with EPSDT paraprofessionals in order to assure conti-
nuity of services.
B. Foster Care "PM 16011 form follow-up and case management
1. Review foster care "PM 160" forms received, initiate appro-
priate follow-up and case management activities.
2. Coordinate activities with Social Workers, providers and
other community agencies involved in services to case to
assure that total health needs are being met.
3. As necessary, make home visits to foster care parents to
interpret atld facilitate follow-up to diagnosis and treat-
ment. Assist in scheduling of appointment and/transportation.
if. Assist in data collection activities. Maintain data,
assure that data collection kept up to date.
`i. Participate in evaluation activity with Program Director,
Deputy Director and Social Service Department Foster Care
Specialist.

I.I. LIAISON AND COORDINATION ACTIVITIES
20% A. In cooperation with Deputy Director, meet with Social Services
Foster Care Liaison to review and update procedures, protocols and
to develop forms, follow-up reporting formats, etc. , designed to
assure that all, vthildren placed in out of home care receive well- .
ness exams consistent with Social Service Department/CHDP Federal
and State guidelines.
B. Assist in the development and implementation of the CHDP/Social
Service Departmerit, foster date component/Interagency Agreement.
C. Assist in development of CHDP Annual Plan with emphasis on the
Foster Care component. '
III. PROGRAM ACTIVITIES
15% A. Attend meetings with Dept. of,-$o.cial Services, State personnel .
and other community agencies to confer, plan, discuss foster care
issues.
B. Represent program at various community activities in advocacy
role for foster il
care chdren when assigned.
C. Provide reports to the MCAH Board when assigned.
IV, PROVIDER RELATIONS
5% A. Participate in outreach and follow-up services with community and
medical resources; e.g., hospital clinic, private providers and
community agencies.
B. At: the direction of the Deputy Director, communicate with CHDP
providers to provide programmatic information related to foster
care children as well as consult and problem solve on case manage-
meant issuer.. .
V. HEALTH EDUCATION AND TRAINING,
8% A. Health education inservices/seminars for Foster Care Parents Asso-
ciation.
B. Participate in health education activities for foster care
caretakers in settings such as clinics, pre-schools/schools/special
education, parenting classes, etc.
C. Assist .in training CHDP/EPSDT and Social Service staff in areas
that pertain to .Foster Care.
VI. OTHER DUTIES AS ASSIGNED
2%,_ A. Assist in interpreting medical guidelines to providers and staff.
B. Assist in audits and perform quality assurance activities in
provider offices.
C. Assist in other foster care activities as deemed appropriate and
necessary.
