Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-928-1 with Diabetics and Endocrinology Specialists, Inc., a corporation, in an amount not to exceed $150,000, to
provide diabetes and endocrinology services to Contra Costa Health Plan (CCHP) members for the period from
February 1, 2016 through January 31, 2018.
FISCAL IMPACT:
This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II (No rate increase)
BACKGROUND:
In April 2014, the County Administrator approved and Purchasing Manager executed Contract #27-928, with
Diabetics and Endocrinology Specialists, Inc. for the period from February 1, 2014 through January 31, 2016, for the
provision of diabetes and endocrinology services for Contra Costa Health Plan members. Approval of Contract
#27-928-1 will allow Contractor to continue providing diabetes and endocrinology services for the period from
February 1, 2016 through January 31, 2018.
