Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-328-8 with Cuong Tat Vu, M.D., Inc., a corporation, in an amount not to exceed $250,000, to provide primary
care services to Contra Costa Health Plan members for the period from October 1, 2015 through September 30, 2017.
FISCAL IMPACT:
This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II. (No Rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized health care services for its members under the terms
of their Individual and Group Health Plan membership contracts with the County.
In January 2014, the County Administrator approved and the Purchasing Services Manager executed Contract
#27-328-7 with Cuong Tat Vu, M.D., Inc., for the period from October 1, 2013 through September 30, 2015,

BACKGROUND: (CONT'D)
to provide primary care services, for Contra Costa Health Plan members.
Approval of Contract #27-328-8 will allow the Contractor to continue to provide primary care services through
September 30, 2017.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, certain specialized professional health care services for its members under the terms
of their Individual and Group Health Plan membership contracts with the County will not be provided.
