Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #26-141-10 with Board of Trustees of Peralta Community College District, an educational
institution, effective January 28, 2015, to amend contract #26-141-8 to modify the Program Plan to add additional
paragraphs to Contractor and County obligations, with no change in the original term of January 1, 2012 through
December 31, 2017.

BACKGROUND:
On January 10, 2012, the Board of Supervisors approved Contract #26-141-8 as amended by Amendment
Agreement #26-141-9 with Board of Trustees of Peralta Community College District for the provision of providing
supervised field instruction experience to provide radiology students with Contra Costa Health Services, for the
period from January 1, 2012 through December 31, 2017.
