Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-918-1 with Gupta Etwaru, M.D., a individual, in an amount not to exceed $145,000, to provide ophthalmology
services to Contra Costa Health Plan (CCHP) members for the period from October 1, 2015 through September 30,
2017.
FISCAL IMPACT:
This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II. (No rate increase)
BACKGROUND:
In November 2013, the County Administrator approved and the Purchasing Services Manager executed Contract
#27-918 with Gupta Etwaru, M.D, for the provision of ophthalmology services to Contra Costa Health Plan members
for the period from October 1, 2013 through September 30, 2015. Approval of Contract #27-918-1 will allow
Contractor to continue providing ophthalmology services through September 30, 2017.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, certain specialized health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County will not be provided.
