Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-977 with Nguyen T. Do, D.O., Inc., a corporation, in an amount not to exceed $350,000, to provide Neurological
Surgery services to Contra Costa Health Plan (CCHP) members, for the period from August 1, 2015 through July 31,
2017.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan Enterprise Fund II.
BACKGROUND:
The Health Plan has an obligation to provide certain specialized medical health care services for its members under
the terms of their Individual and Group Health Plan membership contracts with the County. Under Contract #27-977,
the Contractor will provide Neurological Surgery services to CCHP members through July 31, 2017.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, certain specialized professional health care services for its members under the terms
of their Individual and Group Health Plan membership contracts with the County will not be provided.
