Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#22-986-9 with Monument Impact, a non-profit organization, in an amount not to exceed $435,710, to provide
consulting, technical assistance and program support services to County’s Public Health Division’s Community
Wellness and Prevention Program (CW&PP), for the period from July 1, 2015 through June 30, 2016.
FISCAL IMPACT:
This Contract is funded 69% by State grants, 15% by Foundation grants, 16% by County General funds. (No rate
increase)
BACKGROUND:
This Contract meets the social needs of County’s population by ensuring County and its providers are meeting the
requirements of the United States Department of Agriculture (USDA) and the California Nutrition Network, as well
as, supporting overall injury prevention, nutrition services and obesity programs.
On October 7, 2014, the Board of Supervisors approved Contract #22-986-7 (as amended by Contract Amendment
Agreement #22-986-8) with Monument Impact, for the period from July 1, 2014 through June 30, 2015, for the

BACKGROUND: (CONT'D)

compliance.
Approval of Contract #22-986-9 will allow Contractor to continue to provide services through June 30, 2016.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County will not have access to Contractor’s technical assistance and program support
services that ensure Compliance with USDA and State nutrition guidelines as well as Contractor’s expertise with
regard to injury and obesity prevention.
