Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #22-940-15 with Public Health Foundation Enterprises, Inc., a non-profit corporation,
effective May 1, 2015, to amend Novation Contract #22-940-14, to increase the payment limit by $3,537, from
$223,770 to a new payment limit of $227,307, with no change in the original term of July 1, 2014 through June 30,
2015, and increase the automatic extension payment limit by $884 to a new total of $56,827, and no change in the
term of the automatic extension, through September 30, 2015.
FISCAL IMPACT:
This amendment is funded 15% Federal Title III-C (1) of the Older Americans Act of 1965 and 85% by Federal
Title III-C (2) of the Older Americans Act of 1965, through the Employment and Human Services Department.
(No rate increase)

Subject:Amendment #22-940-15 with Public Health Foundation Enterprises, Inc.
BACKGROUND:
On July 29, 2014, the Board of Supervisors approved Novation Contract #22-940-14 with Public Health
Foundation Enterprises, Inc. for the period from July 1, 2014 through June 30, 2015, which included a three-month
automatic extension through September 30, 2015, for the provision of consultation, technical assistance and
coordination services for County’s Senior Nutrition Program.
Approval of Contract Amendment Agreement #22-940-15 will allow the Contractor to provide additional
consultation, technical assistance, including coordination services through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not provide additional services requested by County’s Senior
Nutrition Program.
