Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #24-680-27 with Telecare Corporation, a corporation, effective May 1, 2015, to amend
Novation Contract #24-680-26, to increase the payment limit by $596,886, from $649,886 to a new payment limit of
$1,246,508, with no change in the original term of July 1, 2014 through June 30, 2015, and to increase the automatic
extension amount by $537,756, from $324,943 to a new total of $862,709, with no change in term of the automatic
extension, through December 31, 2015.
FISCAL IMPACT:
This Amendment is funded 100% by Mental Health Realignment. (No rate increase)
BACKGROUND:
On January 6, 2015, the Board of Supervisors approved Novation Contract #24-680-26 with Telecare Corporation
for the period from July 1, 2014 through June 30, 2015, which included a six-month automatic extension through
December 31, 2015, for the provision of mental health gero-psychiatric services and subacute care for Severely and
Persistently Mentally Ill patients.

BACKGROUND: (CONT'D)
Approval of Contract Amendment Agreement #24-680-27 will allow Contractor to provide additional services to
clients requiring one-on-one supervision, through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, clients will not receive the additional one-on-one supervision as provided by this
Contractor.
