Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#25–012–32 with The Center for Common Concerns, Inc. (dba HomeBase), a non-profit corporation, effective March
1, 2015, to amend Contract #25-012-31, to increase the payment limit by $211,025, from $222,180 to a new payment
limit of $433,205, with no change in the original term of October 1, 2014 through September 30, 2015.
FISCAL IMPACT:
This Amendment is funded 24% by Federal Medi-Cal Administrative Activities (MAA), 30% Behavioral Health
Administration, 12% by budgeted County General funds and 34% by Medi-Cal Administrative Activities/Housing
and Urban Development. (No rate increase)

BACKGROUND: (CONT'D)
1, 2014 through September 30, 2015, for the provision of consultation and technical assistance to the Department
with regard to the Continuum of Care planning and resource development, including grant-writing services for
County’s McKinney-Vento application.
Approval of Contract #25–012–32 will allow the Contractor to provide additional consulting and technical assistance
services to the Behavioral Health Services Division on CountyHomeless Programs through September 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County will not meet all federal guidelines to secure maximum grant funding
required for the homeless programs in Contra Costa County.
