Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#23-522-2 with 5C Consulting Group LLC, a limited liability company, in an amount not to exceed $203,000, to
provide consultation and technical assistance on the Electronic Medical Records Systems, for the period from July 1,
2015 through June 30, 2016.
FISCAL IMPACT:
This Contract is funded 100% by Hospital Enterprise Fund I. (No Rate Increase)
BACKGROUND:
On November 4, 2014, the Board of Supervisors approved Contract #23-522-1, with 5C Consulting, LLC, to provide
consultation and technical assistance to the Department’s Information Systems Director on the Electronic Medical
Records Systems including, but not limited to reporting, data analytics, business intelligence and training, from
October 1, 2014 through June 30, 2015.
Approval of Contract #23-522-2 will allow Contractor to continue providing consultation and technical assistance on
