Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #27-662-6 with Yaron Friedman, M.D., Inc., a corporation, effective May 1, 2015, to amend
Contract #27-662-5, to increase the payment limit by $80,000, from $400,000 to a new payment limit of $480,000,
with no change in the original term of January 1, 2015 through December 31, 2016.
FISCAL IMPACT:
This amendment is funded 100% by Contra Costa Health Plan Enterprise Fund II. (No rate increase)
BACKGROUND:
On January 31, 2015, the Board of Supervisors approved Contract #27-662-5 with Yaron Friedman, M.D., Inc., for
the provision of Obstetrics and Gynecology services for Contra Costa Health Plan (CCHP) patients and county
recipients, for the period from January 1, 2015 through December 31, 2016. At the time of negotiations, the payment
limit was based on target levels of utilization. However, the utilization during the

BACKGROUND: (CONT'D)
term of the agreement was higher than originally anticipated. Services were requested and provided in good faith.
Approval of Contract Amendment Agreement #27-662-6 will allow the Contractor to provide additional obstetrics
and gynecological services to CCHP members through December 31, 2016.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, certain specialized health care services for CCHP members under the terms of
their Individual and Group Health plan membership contracts with the County will not be provided.
