Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County,
Amendment Agreement #24–933–32 with Crestwood Behavioral Health, Inc., effective February 1, 2015, to amend
Contract #24-933-31, to provide services to additional clients at the Eureka facility, with no change in the Contract
Payment Limit of $7,383,000, no change in the original term of July 1, 2014 through June 30, 2015, and no change to
automatic extension through December 31, 2015, in the amount of $3,691,500.
FISCAL IMPACT:
This amendment is funded 100% by Mental Health Realignment and reflects a new rate to add services to clients in
the Eureka facility.
BACKGROUND:
This Contract meets the social needs of County’s population in that it provides subacute skilled nursing care services
for County’s Seriously and Persistently Mentally Ill (SMPI) and neurobehavioral clients.

BACKGROUND: (CONT'D)
Supervisors approved Contract #24-933-31 with Crestwood Behavioral Health, Inc. for the period from July 1, 2014
through June 30, 2015 which included a six-month automatic extension through December 31, 2015, for the
provision of subacute skilled nursing care for County’s Seriously and Persistently Mentally Ill (SPMI) and
Neurobehavioral clients.
Approval of Amendment Agreement #24-933-32, will allow the Contractor to provide services to additional clients at
its Crestwood facility located in Eureka, through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
The Behavioral Health Services Division/Mental Health places clients at Contractor's facilities licensed for various
levels of care. If the amendment is not approved, a significant number of mentally ill young adults and adults may be
displaced to the community without the mental health services they require.
