Approve and authorize the Health Services Director or his designee, to accept, on behalf of the County, Grant Award
#28-876 (#H80CS00050) from the United States Department of Health and Human Services, Health Resources and
Services Administration, to pay the County an amount not to exceed $1,857,667, for the Health Care for the
Homeless Program, for the period from November 1, 2014 through October 31, 2015.
FISCAL IMPACT:
Acceptance of this Grant Award will result in an amount not to exceed $1,857,667 from the U.S. Department of
Health and Human Services, Health Resources and Services Administration (HRSA) for County’s Affordable Care
Act Grants for School-Based Health Centers Capital Program. (County match not required)
BACKGROUND:
The Health Services Department’s Health Care for the Homeless Program has been providing health services to the
homeless populations in Contra Costa County since 1990. The services are provided through designated homeless
ambulatory clinics at the West County Health Center, Antioch Health Center, Concord Respite Health Center and
appointment slots at the Concord

BACKGROUND: (CONT'D)
Health Center Building 2. In addition, the Department runs mobile clinics which provide services to local homeless
shelters, soup kitchens, multi-service centers, detox facilities and other community–based organization. Services
include primary care, substance abuse, mental health, eligibility assistance and other supporting services. The
services are provided to any homeless person residing in the County, regardless of insurance status or ability to pay.
Approval of the Grant Award #28-876 will allow County to accept the funds from the United States Department of
Health and Human Services, Health Resources and Services Administration and expand services to the homeless
population in Contra Costa County, through October 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this award is not approved, the County would not be able to fund this program and without the Program the County
would lose is Federally Qualified Health Center status.
