HomeMy WebLinkAboutMINUTES - 04212015 - C.61RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #26-748-2 with Mauricio Kuri, M.D., Inc., a self-employed individual, effective February 1, 2015, to amend Contract #26-748-1, to increase the payment limit by $130,000 from $240,000 to a new payment limit of $370,000, with no change in the original term of July 1, 2014 through June 30, 2015. FISCAL IMPACT: This amendment is funded 100% by Hospital Enterprise Fund I. (No rate increase) BACKGROUND: On May 13, 2014, the Board of Supervisors approved Contract #26-748-1 with Mauricio Kuri, M.D., for the period from July 1, 2014 through June 30, 2015, for the provision of plastic surgery services including consultation, training, on-call services and medical and/or surgical procedures.
