Approve and authorize the Health Services Director, or his designee to execute on behalf of the County, Contract
#27-253-11 with California Eye Clinic, a partnership, in an amount not to exceed $650,000, to provide ophthalmology
and optometry services for Contra Costa Health Plan members and County recipients for the period from May 1,
2015 through April 30, 2017.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan Enterprise Fund II. Costs depend upon utilization. (No
rate increase)
BACKGROUND:
On June 4, 2013, the Board of Supervisors approved Contract #27-253-10 with California Eye Clinic, a partnership,
for the period from May 1, 2013 through April 30, 2015 to provide ophthalmology and optometry services to Contra
Costa Health Plan members and County recipients. Approval of Contract #27-253-11 will allow the Contractor to
continue to provide ophthalmology and optometry services through April 30, 2017.
