Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment/Extension Agreement #23-478-8 with Santa Rosa Consulting, Inc., a corporation, effective September 1,
2014, to amend Contract #23-478-7 to modify the payment provisions with no change in the original payment limit
of $1,500,000 and to extend the term from June 30, 2015 to through June 30, 2016.
FISCAL IMPACT:
This amendment is funded 100% by Enterprise Fund I. (Adding provision of $40,000 for travel reimbursement)
BACKGROUND:
On January 6, 2015, the Board of Supervisors approved Contract #23-478-7, with Santa Rosa Consulting, Inc., for
the period from September 1, 2014 through June 30, 2015, for the provision of consulting, technical support, training
and project management to the Department’s Information Systems Unit for County’s computer systems. Due to a
mutual mistake, the Division and Contractor did not include provisions for travel reimbursement needed on behalf of
the County through the term of this Contract. Approval of Contract Amendment Agreement #23-478-8 will add
provisions for travel expenses with no increase in the total payment limit and allow the Contractor to continue to
provide services through June 30, 2016.
