Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#24-086-145(11) with Crestwood Behavioral Health, Inc., a corporation, in an amount not to exceed $95,000, to
provide e mergency residential care placement services to mentally ill adults, for the period from January 1, 2015
through December 31, 2015.
FISCAL IMPACT:
This Contract is funded 100% by Mental Health Realignment. (No rate increase)
BACKGROUND:
On January 7, 2014, the Board of Supervisors approved Contract #24-086-145(10) with Crestwood Behavioral
Health, Inc., for the period from January 1, 2014 through December 31, 2014, for the provision of emergency
residential care.

BACKGROUND: (CONT'D)
of Contract #24-086-145(11) will allow the Contractor to continue to provide emergency residential care services
through December 31, 2015. This contract includes modifications to the standard indemnification provisions
including mutual.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, there will be fewer emergency residential facilities to provide services to SSI/SSP
eligible Clients in the Continuing Care Program.
