Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
#27-578-8 with Dialysis Access Center Inc., a corporation, in an amount not to exceed $350,000 to provide dialysis
services to Contra Costa Health Plan members for the period from April 1, 2015 through March 31, 2017.
FISCAL IMPACT:
This Contract is funded Contra Costa Health Plan 100% Enterprise Fund III. This contract reflects a rate increase.
BACKGROUND:
On April 9, 2013, the Board of Supervisors approved Contract #27-578-7 with Dialysis Access Center Inc. to provide
professional dialysis services for Contra Costa Health Plan members for the period from April 1, 2013 through March
31, 2015.
Approval of Contract #27-578-8 will allow the Contractor to continue providing services through March 31, 2017.
