Approve and authorize the Health Services Director, or designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-433-5 with Michael Levin, M.D., a self-employed individual, effective January 2, 2015,
to increase the total Contract Payment Limit by $17,640 from $232,960 to a new total payment limit of $250,600, to
provide additional services and travel on behalf of the County with no change in the original term of January 1, 2015
through December 31, 2015.
FISCAL IMPACT:
This Amendment is funded 100% by Mental Health Realignment. (rate increase)
BACKGROUND:
For a number of years the County has contracted with Medical, Dental and Mental Health Specialists to provide
specialized professional services, which are not otherwise available.
On December 2, 2014, the Board of Supervisors approved Contract #74-433-4 with Michael Levin, M.D. for the
provision of professional outpatient psychiatric services and expert testimony in conservatorship trials, for the period

BACKGROUND: (CONT'D)

Approval of Contract Amendment #74-433-5 will allow the Contractor to provide additional services including travel
on behalf of the County, through December 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County’s clients will not have access to Contractor’s outpatient psychiatric
services, which may result in a reduction in the overall levels of service to the community.
