HomeMy WebLinkAboutMINUTES - 12162014 - C.86RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #74-271-52(7) with Heftsi Assaf, MFT, a self-employed individual, effective November 1, 2014, to amend Contract #74-271-52(6), to make technical adjustments to the rate sheet and increase the payment limit by $5,000, from $110,000 to a new payment limit of $115,000, with no change in the original term of July 1, 2013 through June 30, 2015. FISCAL IMPACT: This amendment is funded 50% by Federal Medi-Cal and 50% State General Fund. (Rate increase) BACKGROUND: On August 13, 2013, the Board of Supervisors approved, Contract #74-271-52(6) with Heftsi Assaf, MFT for the provision of Medi-Cal specialty mental health services, for the period from July 1, 2013 through June 30, 2015. Approval of Contract Amendment Agreement #74-271-52(7) will allow the Contractor to provide services to additional Medi-Cal beneficiaries through June 30, 2015. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, Contractor will not be able to provide services to additional Medi-Cal beneficiaries resulting in a lower level of services to the community. CHILDREN'S IMPACT STATEMENT: Not Applicable.
