Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #74-341-30(7) with Umoja Counseling and Consulting Services, Karen J. Benjamin,
Licensed Clinical Social Worker, Inc., a corporation, effective November 1, 2014, to increase the payment limit by
$50,000 from $200,000 to a new payment limit of $250,000, to make technical adjustments to the rate sheet, with no
change in the original term of July 1, 2013 through June 30, 2015.
FISCAL IMPACT:
This amendment is funded 100% by Federal Medi-Cal and 50% State General Funds. (Rate increase)
BACKGROUND:
On August 6, 2013, the Board of Supervisors approved Contract #74-341-30(6), with Umoja Counseling and
Consulting Services, Karen J. Benjamin, Licensed Clinical Social Worker, Inc., for the period from July 1, 2013
through June 30, 2015, for the provision of Medi-Cal specialty mental health services.

BACKGROUND: (CONT'D)
County requested and Contractor agreed to provide additional services to Medi-Cal beneficiaries, therefore, County
has agreed to modify the rate sheet and increase the total payment limit of the Contract.
Approval of Contract Amendment Agreement #74-341-30(7) will allow the Contractor to provide services to
additional Medi-Cal beneficiaries through June 30, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be able to provide services to additional Medi-Cal
beneficiaries resulting in a lower level of services to the community.
