Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County as follows:
1. Cancellation Agreement #26 768-2 with Fred Nachtwey, M.D., a self-employed individual, effective on the close
of business on October 31, 2014; and 2. Contract #26-768-3 with Fred Nachtwey, M.D., a self-employed individual,
in an amount not to exceed $200,000, to provide professional pulmonary services at Contra Costa Regional Medical
Center and Contra Costa Health Centers (CCRMC), for the period from November 1, 2014 through October 31,
2015.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I. (Adding rate for administrative services)
BACKGROUND:
On June 3, 2014 the Board of Supervisors approved Contract #26-768 with Fred Nachtwey, M.D. for the provision of
professional pulmonary services including, consultation, and medical procedures at CCRMC, through June 30, 2017.
Due to additional pulmonary services including administrative duties, the Division terminated Contract #26-783
effective October 31, 2014, on Contract

BACKGROUND: (CONT'D)
termination #26-768-2.
The Department and Contractor have agreed to a mutual cancellation of Contract #26-768 in accordance with
General Conditions, Paragraph 5. (Termination and Cancellation). Approval of Cancellation Agreement #26-768-2
will accomplish this termination.
Contract #26-768-3 will allow Contractor to provide more clinical services, including administrative duties with an
effective date of November 1, 2014.
Approval of Contract #26-768-3 will allow the Contractor to provide professional pulmonary services, consultation,
training, and medical procedures including Administrative duties at CCRMC through October 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients requiring professional pulmonary services at CCRMC will not have access to
Contractor’s services, which may result in a reduction in the levels of service to the community.
