HomeMy WebLinkAboutMINUTES - 12022014 - C.124RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment/Extension Agreement #26-771-1 with Health Leads USA, a corporation, effective July 1, 2014, to amend Contract #26-771, to increase the payment limit from $0, to a new payment limit of $310,000, and extend the term from May 31, 2016 through May 31, 2017. FISCAL IMPACT: This Contract is funded 100% by a Grant from Kaiser Community Benefit Foundation. This amendment will allow the Contractor to be paid monthly for services starting in the second year of this Agreement. BACKGROUND: On July 29, 2014, the Board of Supervisors approved Contract #26-771, with Health Leads USA to develop and track patients needing non-medical resources by implementing a health leads desk at the West County Health Center for data gathering of patients needing assistance in accessing food, childcare, energy bill assistance, and provide residential placement services through May 31, 2016. BACKGROUND: (CONT'D) Contract Amendment/Extension Agreement #26-771-1 will allow the Contractor to provide additional services, be reimbursed in the second and third year, modify the certain provisions of the Agreement as mutually agreed by County and Contractor and extend the term through May 31, 2017. Contract execution is contingent on review and approval by County Counsel as to form. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, Contractor will not continue providing services. CHILDREN'S IMPACT STATEMENT: Not applicable.
