Approve and authorize the Purchasing Agent on behalf of the Health Services Department to execute a change to
Purchase Order #46528 with Pharmedium Services LLC, to add $80,000 from $225,000 to a new total of $305,000,
for pain management/pre-mix medications for the Contra Costa Regional Medical Center and Contra Costa Health
Centers, for the period September 1, 2014 to January 31, 2015.
FISCAL IMPACT:
100% Funding is included in the Enterprise Fund l Budget.
BACKGROUND:
Recalls and back orders of medications have prompted the Pharmacy Department to directly purchase medications
through manufacturers such as Pharmedium Services, LLC. Pharmedium Services, LLC strictly adheres to U.S. Food
and Drug Administration (FDA) compounding guidelines through exclusive use of commercially available FDA
approved sterile additives, solutions and containers. These
medications are in short supply or are not commercially available through our contracted drug wholesaler,

BACKGROUND: (CONT'D)
Cardinal Health.
This change increases the amount of money on the purchase order to cover the cost of these medications through
January 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this change of order is not approved, Contra Costa Regional Medical Center and Health Centers will not be able to
purchase the medications required to appropriately manage/control acute pain in primarily post-operative and OB
patients.
