Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment/Extension Agreement #23-529-2, to amend Contract #23-529-1 with Relyon Consulting, Inc., a
corporation, to extend the term from September 30, 2014 through January 31, 2015 and to increase the payment limit
by $100,000 from $292,000 to a new total payment limit $392,000.
FISCAL IMPACT:
This amendment/extension is funded 100% by Enterprise Fund I. (No rate increase)
BACKGROUND:
On October 15, 2013, the Board of Supervisors approved Contract #23-529-1, with Relyon Consulting, Inc., for the
provision of consultation and technical assistance to the Information Technology Unit of the Health Services
Department with regard to support of the electronic health record (EHR) project, for the period from October 1, 2013
through September 30, 2014.

Agreement #23-529-2 will allow the Contractor to continue providing services through January 31, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this extension is not approved, the Department will not have the expert technical assistance available for support
and optimization of the EHR project or for a successful major version upgrade.
