Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract
Amendment Agreement #22-939-21 with Food Bank of Contra Costa and Solano, a non-profit corporation, effective
August 1, 2014, to amend Contract #22-939-20, to increase the payment limit by $75,000, from $71,337 to a new
payment limit of $146,337, with no change in the original term of May 1, 2014 through February 28, 2015.
FISCAL IMPACT:
This amendment is funded 100% by the Ryan White HIV Treatment Modernization Act. (No rate increase)
BACKGROUND:
In March, 2011, the County Administrator approved and Purchasing Manager executed, Contract #22-939-20 with
Food Bank of Contra Costa County for the period from March 1, 2014 through February 28, 2015, for the
coordination of food services for County residents diagnosed with HIV and their families.

BACKGROUND: (CONT'D)
of Contract #22-939-21 will allow the Contractor to provide additional services including purchasing additional
protein and produce for County residents with HIV, through February 28, 2015.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, Contractor will not be able to provide sufficient protein and fresh produce to the existing
caseload of County residents, who are diagnosed with HIV.
