HomeMy WebLinkAboutMINUTES - 10212014 - C.74RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #26-672-3 with Siri Sunderi Cheng, M.D., a self-employed individual, effective August 1, 2014, to amend Contract #26-672-2, to provide an additional level of services to include clinical and administrative support services, at a rate of $150 per hour with no change in the payment limit of $1,065,000 and no change in the original term of October 1, 2013 through September 30, 2016. FISCAL IMPACT: This Amendment is funded 100% by Enterprise Fund I. It includes an additional level of services at the rate of $150 per hour for clinical and administrative support services. BACKGROUND: On December 3, 2013, the Board of Supervisors approved Contract #26-672-2 with Siri Sunderi Cheng, M.D., for the provision of professional otolaryngology services at Contra Costa Regional Medical Center and Contra Costa Health Centers (CCRMC), for the period from October 1, 2013 through September 30, 2016. Approval of Contract Amendment Agreement #26-672-3 will allow the Contractor to provide additional level of services at CCRMC through September 30, 2016. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, there may be a reduction in the level of otolaryngology services Contractor provides. CHILDREN'S IMPACT STATEMENT: Not applicable.
