HomeMy WebLinkAboutMINUTES - 10212014 - C.72RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment/Extension Agreement #26-663-5 with Dino Elyassnia, M.D., A Professional Corporation, effective October 31, 2014, to amend Contract #26-663-4, to increase the payment limit by $60,000, from $480,000, to a new payment limit of $540,000, and extend the term from October 31, 2014 through January 31, 2015. FISCAL IMPACT: This Amendment is funded by Enterprise I Budget. Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services. (No rate increases) BACKGROUND: On October 23, 2012, the Board of Supervisors approved Contract #26-663-4 with Dino Elyassnia, M.D., A Professional Corporation, for the period from November 1, 2012 through October 31, 2014, to provide plastic surgery services including clinic coverage, consultation and medical and/or surgical procedures, for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers. CONSEQUENCE OF NEGATIVE ACTION: If this amendment is not approved, patients requiring these medical services at Contra Costa Regional Medical and Contra Costa Health Centers would have longer wait times and possibly go untreated.
